[摘要] 目的 观察不同时相应用无创呼吸机治疗慢性阻塞性肺疾病(COPD)的临床疗效。方法 选择100例COPD患者，随机分为例数相等的夜间组与白昼组，两组均给予基础治疗，夜间组于夜间加用无创呼吸机进行治疗，白昼组则在白天进行，对两组治疗前后呼吸困难症状评分、肺功能、C-反应蛋白(CRP)及脑钠肽(BNP)水平进行比较及统计学分析。 结果 夜间组及白昼组治疗后肺功能改善差异无统计学意义(P > 0.05)；呼吸困难症状评分、CRP及BNP水平改善值差异有统计学意义(P < 0.05)。 结论 夜间应用无创呼吸机治疗COPD，更有助于减少COPD急性发作的次数，从而改善患者的生活质量。

Clinical analysis of noninvasive ventilator in different time phase in the treatment of chronic obstructive pulmonary disease

[Abstract] Objective To observe the clinical effect of noninvasive ventilator in different time phase in the treatment of chronic obstructive pulmonary disease (COPD). Methods One hundred COPD patients were chosen and randomly divided into night group and day group, with same cases in each group. Both groups were given routine treatment, the night group was given noninvasive ventilator at night, while the day group was carried on in the daytime. The symptom scores of dyspnea, lung function, creactive protein (CRP) and brain natriuretic peptide (BNP) before and after treatment between the two groups were compared and statistically analyzed. Results The differences of lung function improvement after treatment between night group and day group were no statistically significant (P > 0.05). There were statistically significant differences in the improvement of symptom scores of dyspnea, the level of CRP and BNP of the two groups (P < 0.05). Conclusion The application of noninvasive ventilator at night in the treatment of COPD can help to reduce the acute attack times of COPD, thus to improve the quality of life of patients.
