主动电极高位室内间隔起搏临床观察

     [摘要] 目的:对主动电极高位室内间隔起搏的疗效进行观察和总结。方法:以2005年7月~2010年7月来本院行心脏起搏器植入的124例患者作为观察对象,根据心室电极固定部位的不同将他们分为A、B、C、D四组,在置入电极、脉冲发生器后测量有关参数,并在术后1、3、6个月进行随访,同时请患者填写生活质量调查问卷。结果:A、B、C三组患者各项血流动力学指标以及生活质量出现下降趋势,而D组患者的相关指标和生活质量均未出现明显下降(P<0.05),差异具有统计学意义,同时D组的治疗效果也要优于A、B、C三组(P<0.05),差异有统计学意义。结论:应用主动电极高位室内间隔起搏能够有效改善患者的心脏功能和血流动力学,是一种非常理想的起搏治疗方式。

    Clinical observation of active electrode high indoor interval follow-up

    [Abstract] Objective: Internal partitions on the active electrode high efficacy of pacing were observed and summarized. Methods: July 2005 to July 2010 in our hospital 124 patients implanted pacemaker patients as observed object, according to the fixed parts of the different ventricular leads them into A, B, C, D four groups. In the implanted electrode, after measuring the relevant parameters of the pulse generator, and 1, 3, 6 months after surgery were followed up at the same time please fill in the quality of life questionnaire in patients. Results: A, B, C of the three group's patients hemodynamics and quality of life declining trend, while the D group were related to indicators and quality of life was no obvious decrease (P<0.05), with statistical significance, while D group, the treatment group also better than the A, B, C three groups (P<0.05), with significant differences. Conclusion: The high active electrode pacing interval can improve the indoor patients cardiac function and hemodynamics, the pacing is a very good treatment.

    生理起搏就是指人工心脏起搏器在对患者的心率进行矫正时,通过对不同的起搏方式、不同的电极位置以及各种间期的调整方法的应用来使血流动力学效果和起搏节律更加接近心脏原有的生理状态[1]。心脏起搏器对于患者心脏血流动力学的影响得到了越来越多专家和学者的关注,而采用接近生理性或生理性的起搏已经成为近年来心脏起搏治疗的发展趋势,本文主要通过探讨和分析不同起搏部位对双心室血流动力学的影响,来对主动电极高位室内间隔起搏的疗效进行观察和总结。
