二甲双胍联合炔雌醇环丙孕酮对多囊卵巢综合征不孕症患者性激素水平及妊娠结局的影响(1)
【摘要】 目的 探討二甲双胍联合炔雌醇环丙孕酮对多囊卵巢综合征不孕症患者性激素水平及妊娠结局的影响。方法 86例多囊卵巢综合征不孕症患者， 随机数字表法分为对照组和观察组， 各43例。
对照组采用炔雌醇环丙孕酮治疗， 观察组采用二甲双胍联合炔雌醇环丙孕酮治疗， 两组均治疗3个月。
对比两组性激素水平及治疗后排卵率、妊娠率。结果 两组治疗前黄体生成素(LH)、卵泡刺激素(FSH)、胰岛素(INS)水平对比， 差异无统计学意义(P>0.05)；治疗后两组LH、FSH、INS水平较治疗前明显降低(P<0.05)， 治疗后观察组LH、INS水平低于对照组(P<0.05)， 两组FSH水平比较差异无统计学意义(P>0.05)；两组治疗后进行6个月随访， 观察组排卵37例， 妊娠20例， 排卵率为86.05%， 妊娠率为46.51%；对照组排卵27例， 妊娠10例， 排卵率为62.79%， 妊娠率为23.26%。观察组患者排卵率及妊娠率均明显高于对照组， 差异有统计学意义(P<0.05)。结论 二甲双胍联合炔雌醇环丙孕酮治疗多囊卵巢综合征不孕症， 改善患者性激素水平， 提高妊娠率， 值得推广。

【Abstract】 Objective To investigate influence by metformin combined with ethinylestradiol-cyproterone acetate on hormone level and pregnancy outcome in polycystic ovarian syndrome infertility patients. Methods A total of 86 patients with polycystic ovarian syndrome infertility were divided by random number table into control group and observation group， with 43 cases in each group. The control group received ethinylestradiol-cyproterone acetate for treatment， and the observation group received metformin combined with ethinylestradiol-cyproterone acetate for treatment. Treatment for both groups lasted for 3 months. Comparison was made on sex hormone level， ovulation rate and pregnancy rate after treatment between the two groups. Results Before treatment， there was no statistically significant difference of luteinizing hormone (LH)， follicle-stimulating hormone (FSH)， and insulin (INS)levels between the two groups (P>0.05). After treatment， both groups had obviously lower LH， FSH and INS levels than those before treatment (P<0.05)， and the observation group had lower LH and INS levels than the control group (P<0.05)， while their difference of FSH had no statistical significance (P>0.05). Follow-up lasted for 6 months in both groups. The observation group had 37 ovulation cases and 20 pregnancy cases， with ovulation rate as 86.05% and pregnancy rate as 46.51%.
