凝血、抗凝及纤溶指标检测在冠心病、脑梗死中的表达及意义(1)

     【摘要】 目的 探讨凝血、抗凝及纤溶指标检测在冠心病、脑梗死中的表达及意义。方法 80例冠心病患者(冠心病组)和110例脑梗死患者(脑梗死组)以及80例接受体检的健康者(健康对照组)为研究对象;其中冠心病组又分为急性心肌梗死(AMI)组21例、不稳定型心绞痛(UA)组32例、稳定型心绞痛(SA)组27例, 脑梗死组又分为进展组69例、完全组41例。比较所有入选者凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fg)、D-二聚体(D-D)、血小板计数(PLT)、组织型纤溶酶原激活物(t-PA)水平。结果 冠心病组、脑梗死组Fg、D-D明显高于健康对照组, PLT、t-PA明显低于健康对照组, 差异具有统计学意义(P<0.05); AMI组、UA组、SA组、健康对照组Fg、D-D依次降低, PLT、t-PA依次升高, 进展组、完全组、健康对照组Fg、D-D依次降低, PLT、t-PA依次升高, 差异具有统计学意义(P<0.05)。结论 冠心病、脑梗死患者的Fg、D-D、PLT、t-PA水平异常, 机体处于高凝血、低抗凝和纤溶紊乱状态, 临床上可根据Fg、D-D、PLT、t-PA表达判断血栓情况。

    Expression and significance of blood coagulation, anticoagulation and fibrinolysis detection in coronary heart disease and cerebral infarction WANG Hong-gang. Weifang City People’s Hospital, Weifang 261041, China

    【Abstract】 Objective To investigate expression and significance of blood coagulation, anticoagulation and fibrinolysis detection in coronary heart disease and cerebral infarction. Methods There were 80 coronary heart disease patients (coronary heart disease group), 110 cerebral infarction patients (cerebral infarction group), and 80 healthy people (healthy control group) as study subjects. The coronary heart disease group was further divided into acute myocardial infarction (AMI) group with 21 cases, unstable angina (UA) group with 32 cases and stable angina (SA) group with 27 cases. The cerebral infarction group was divided into progress group with 69 cases and complete group with 41 cases. Comparisons were made on prothrombin time (PT), activated partial thromboplastin time (APTT), fibrinogen (Fg), D-Dimer (D-D), platelet count (PLT), and tissue-type plasminogen activator (t-PA) levels in all patients. Results The coronary heart disease group and cerebral infarction group had obviously higher Fg, D-D, and lower PLT, t-PA than the healthy control group. Their differences had statistical significance (P<0.05). From AMI group, UA group, SA group, to the healthy control group, Fg and D-D were gradually decreased, PLT and t-PA were gradually increased. From the progress group, the complete group to healthy control group, Fg and D-D were gradually decreased, PLT and t-PA were gradually increased. Their differences had statistical significance (P<0.05). Conclusion Coronary heart disease and cerebral infarction patients have abnormal levels of Fg, D-D, PLT and t-PA, along with status of high blood coagulation, low anticoagulation, and disorder fibrinolysis.
