【摘要】 ] 研究背景 保乳手术的目的是在取得同根治手术相同的生存率和较好的局部控制率之外，获得最佳的乳房形态、减少心理创伤、提高生活质量。而诸多因素：与病人相关的、与肿瘤相关的以及与治疗相关的将可能影响保乳术后乳房形态。仍有相当一部分患者乳房形态存在严重畸形，需要手术来修复。 目的 对保乳术后乳房局部缺损畸形进行分类，寻找最佳修复时机和修复方法。 方法 针对保乳术后乳房部分缺损的临床表现，采用相应的修复手段，包括 局部皮瓣或邻位皮瓣法、背阔肌肌瓣或肌皮瓣带蒂转移、横形腹直肌肌皮瓣、腹壁下动脉穿支皮瓣吻合血管游离移植，及对侧乳房的矫正。 结果 保乳术后 仍有相当一部分患者，需要手术来修复， 其中患者的年龄、体重指数、绝经状况、乳房大小、肿物直径、肿物位于外上象限时的切口数量、切除次数、总的放疗剂量、纤维化、手术方式对保乳术后乳房形态及重建效果有明显影响。 结论 保乳手术是乳腺癌外科治疗划时代的进步，带给患者的是划时代的解放，而整形外科顺应患者的要求，提供相应的修复手段，最终保证了保乳手术的初衷，体现了乳腺癌多学科综合治疗的优势和必要性。

【 Abstract 】 Background The subject of breast-conservating surgery (BCT) is to obtain the same equivalent long- term survival and oncologic local control as modified radical mastectomy, furthermore the benefits of BCT are to maintain cosmetic results .It also decrease psychological drawback ,improve life quality. But there are a lot of factors impact the final shape of the breast after BCT, such as the patient demographics, tumour characteristics, and correlated therapy. There are numerous patients with comparatively severe deformity following BCT , options available for reconstructing partial mastectomy defects. Object To classify the partial deformity post breast conservation therapy, to state how to choice the suitable methods for different cases. Methods It's very important to analyze the patients' condition . Several methods such as transferring local flap, m yocutaneous latissimus dorsi flap, TRAM, free DIEP can be used. Also the opposite breast can be modified when it's necessary. Results A lot of factors of patient such as age, bady weight index, menopause, size of breast, diameter of tumour, number of partial mastectomy located upper lateral quadrant, doze of radiation ,fibrosis degree affected the final results. Conclusion Breast-conservating therapy is historical evolved in breast cancer surgical therapy. It benefits a lot for patients. Plastic surgery services the demand of patients, provides the reconstructive methods, ensures the original intention of BCT, personalizes the advantage and necessary of multi-treatment for breast cancer.
