【摘要】 目的 探讨高频超声及彩色多普勒血流显像对腮腺肿瘤性病变的诊断及鉴别诊断的意义。方法 回顾分析我院彩超室检出并经手术病理证实的75例腮腺实性肿瘤，其中良性肿瘤49例，恶性26例。应用高频超声观察肿瘤的形态、边缘及内部回声特点，彩色多普勒血流显像(CDFI)观察肿瘤内血流信号特点，频谱多普勒测肿瘤血流信号频谱。结果 腮腺良性肿瘤多数形态规则、边缘清楚、内部回声均匀，CDFI显示无血流信号或点状、细线状血流信号；恶性肿瘤则多数边界不清，呈不规则浸润状，内部光点分布不均，CDFI多数可显示条状、树枝状分布血流信号，收缩期峰值血流速度较高。结论 高频超声及彩色多普勒对腮腺实性肿瘤具有良好的诊断、鉴别诊断价值。

    【Abstract】 Objective To evaluate the value of high-frequency ultrasound and color Doppler flow imaging in diagnosis of tumor in the parotid gland.Methods The ultrasound findings of 75 cases of parotid tumors proved by operation and pathology,including 49 cases of benign tumors and 26 of malignant ones were retrospectively analysed.High-frequency ultrasound was used to observe the shape,boundary and internal echo texture of tumors,color Doppler flow imaging(CDFI) was added to observe blood flow signal,peak systolic velocity and resis index were measured by pulsed wave Doppler.Results Benign tumors of parotid gland were mostly regular in shape,clear in boundary,and with a regular internal echo texture,CDFI showed none blood flow signal or spot-like,stick-like blood flow signal.While the malignant ones were mostly irregular in shape,unclear in boundary,and with a irregular internal echo texture,most of which CDFI showed stick-like,brunch-like blood flow signal.Pulsed wave Doppler also showed higher peak systolic velocities than the benign ones.Conclusion The high-frequency ultrasound and color Doppler flow imaging are of significant value in diagnosis,especially in differentiating diagnosis of benign and malignant tumors of parotid gland. ......
