【摘要】 目的 研究上海市肥胖中小学生出现代谢综合征(MS)病理变化的状况，论证代谢综合征从儿童青少年抓起的重要性。 方法 对30例肥胖学生的糖代谢、脂肪代谢、肝脏脂肪沉积、肝功能变化、眼底血管变化等进行检查和分析，并与对照组比较。 结果 在肥胖学生中存在:(l)糖代谢紊乱。肥胖学生中存在胰岛素抵抗(IR)，空腹血胰岛素(Fins)、胰岛素抵抗指数(HOMA-IR)升高。(2)脂肪代谢紊乱。肥胖学生中存在血脂异常的占23.33%(7/30)，而对照组为0;表现为甘油三酯(TG)升高、高密度脂蛋白-胆固醇(HDL-C)降低。(3)肝脏脂肪沉积和肝功能变化。肥胖学生出现肝脏脂肪细胞浸润的占23.33%(7/30)，轻度脂肪肝或脂肪肝占13.33%(4/30)，对照组为0;肥胖组ALT数值升高，但两组ALT异常率在统计学上无差异。(4)出现眼底血管早期病理变化的占30.00%(9/30)，动静脉比例异常、静脉迂曲变粗的分别占13.33%(4/30)、16.67%(5/30)，而对照组为0。 结论 学生肥胖可以导致机体多种代谢紊乱，相应指标出现变化，并随年龄增大而加重，直至最终出现各系统的病变。

    【Abstract】 Objective To investigate the metabolic syndrome(MS)among obese students in primary and middle schools of Shanghai and show that preventing MS among children and adolescents is important.Methods To find out the disorders of glucose and lipids metabolism，liver fat deposit，liver function abnormal and the pathological changes of blood vessel on eye ground among30obese students and30age and sex matched controls.Results Obese students showed:(1)The disorder of glucose metabolism.The prevalence of insulin resistance(IR)was significant.The means of fasting insuline(Fins)level and HOMA-IR valu were higher.(2)The disorder of lipids metabolism.Seven of30(23.33%)had abnormal serum lipids level.It was normal in controls.The means of TG was higher and HDL-C was lower.(3)Liver fat deposit and liver function abnormal.Eleven of30(23.33%)had liver fat deposit ......
